The Housing Stability division of Human Services and Public Health is seeking a full time Case Management Assistant to join the Housing Stability Streets to Housing team. This employee will specialize in connecting people experiencing unsheltered homelessness with treatment and recovery services. This individual will create and cultivate relationships within the Substance and Opioid Use (SUD/OUD) space to ensure the Streets to Housing team is able to effectively triage and navigate housing stability and substance use support options.
Location and hours:
This position is hybrid and will be performed both on-site at various locations throughout the community and remote as job duties require. Work hours will be Monday through Friday, with flexible hours between 7:00 a.m. to 5:00 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
In this position, you will:
Provide comprehensive wrap-around services to individuals with OUD and any co-occurring SUD/mental health (MH) conditions, including housing, transportation, education, job placement, job training, or childcare.
Participate in street and site-based triage and navigation activities to better understand and support the detox, recovery and chemical health needs of clients served by Streets to Housing.
Develop and cultivate relationships with public health and community-based providers that offer SUD/OUD specific services to ensure warm hand offs to programs.
Complete Administration activities including program and community meetings, data entry, and other assessments and documentation as identified.
Need to have:
One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Nice to have:
Experience:
Serving individuals living with substance use disorder and/or a mental health condition.
Working in a health care setting, such as a clinic, hospital, or walk-in center.
Effectively interacting with people from diverse backgrounds including efforts towards disparity reduction.
Knowledge of:
County and community resources for mental health, substance use disorder, social services, and basic needs.
County and state record systems.
Ability to:
Function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgment.
Organize information and assist with maintaining an efficient daily workflow.
Work well in a group setting, be accountable to the team.
