The Family Health area within Hennepin County Public Health has two openings for a bilingual Public Health Nurse Family Home Visitor. One position will work for the Health Families America (HFA) Program and requires the candidate to be bilingual in Spanish. The second position will primarily provide postpartum newborn home visiting in the evidence-based Family Connects program and requires the candidate to be bilingual in Spanish, Somali or Hmong.

In these positions, you will:
Deliver family home visiting services, in accordance with model and funding requirements, and stay current with changes to rules, requirements and model standards.
Develop therapeutic relationships, utilize concepts of reflection and motivational interviewing with caregivers and their families in a home visiting environment.
Analyze assessed data and assist individuals to identify relevant goals along with specific supports that will enhance their ability to meet goals.
Develop and maintain community relationships to support client referrals and link families with community resources that are relevant to their specific needs.
Maintains appropriate, thorough, timely and compliant documentation on visits into an electronic database.
Ensure services are resident-centered, culturally appropriate, and equity focused.
Need to have:
Licensure as a Registered Nurse issued by the state of Minnesota.
Certification as a Public Health Nurse by the Minnesota Board of Nursing.
A bachelor's degree or higher in nursing from an accredited school of nursing and six months or more of full-time equivalent work experience in a related area.
A valid driver's license and daily access to reliable transportation to travel within and outside the metro area to meet with people in their residential settings and/or perform authorized off-site work duties.
Nice to have:
One or more years of experience providing family home visiting services.
Experience working within a team, organization, or community to reduce health disparities.
Experience providing evidence-based home visiting and training in one of the following models: Nurse Family Partnership, Healthy Families America, and/or Family Connects.
The HFA position requires the PHN to be bilingual in Spanish. The Family Connects position has a preference for a bilingual home visitor who speaks Spanish, Somali, or Hmong. Bilingual candidates who pass a language assessment may qualify for additional differential bilingual pay and duties.
