The Hennepin County Human Services and Public Health's (HSPH) Transition Age Young Adult team (TAYA) in the Behavioral Health Case Management (BHCM) area is seeking a Senior Social Worker with experience in working with young adults (ages 17-26) with serious and persistent mental illness (SPMI) and substance use disorders. TAYA provides interdisciplinary case management services for young adults with SPMI and/or substance use disorders. A person-centered, recovery approach is the model utilized in Behavioral Health and HSPH. BHCM staff providing case management must meet the requirements for mental health practitioner or mental health professional in MN Statutes 245.4871.
A Senior Social Worker on a Transition Age Young Adult Team provides intensive social work services to young adults with complex biopsychosocial conditions requiring individualized, comprehensive, culturally competent assessment, service planning, referral/linkage, monitoring, and advocacy. A knowledge base of serious and persistent mental illness, chemical dependency, recovery, and community resources is preferred. This position is a part of an interdisciplinary team assisting individuals in achieving and maintaining stability and recovery in the most integrated setting possible in the community.
Effective January 1, 2023, there will be a general salary adjustment of 2.5% to the minimum and maximum of the range.
Location and hours:
This position is hybrid and will be performed both on-site at various community sites throughout Hennepin County and remote as job duties require. Work hours are Monday through Friday 8 a.m. to 4:30 p.m. with room for flexibility. Occasional evening hours may be required. While this position is designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position you will:
Provide mental health targeted case management to individuals at risk or subject to civil commitment due to mental illness, chemical dependency, or mental illness.
Engage individuals in treatment plans incorporating their strengths, resources, and goals to ensure health, safety, and access to the best level of care and independence in the community; collaborate with Children’s Mental Health during the transition of services.
Work closely with a wide range of community, County, and State partners to assess, intervene, and resolve a variety of time-sensitive financial, benefit, clinical, and provider barriers to facilitate individual discharge and recovery plans.
Assess the needs with a person-centered and culturally competent approach to determine a plan of action that arrives from Motivational Interviewing and stage-based intervention.
Work collaboratively with the interdisciplinary team and effectively with in-patient treatment teams, community providers, and families.
Need to have:
One of the following:
Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles or social welfare.
Bachelor's degree in social work, psychology, sociology, or human services, and at least 27 quarter credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
A valid driver's license and daily access to reliable transportation to travel primarily within/outside of the metro area to transport or meet with individuals in their residential settings and/or perform authorized off-site work duties.
Ability to lift up to 25 pounds with or without reasonable accommodation.

Working with adult individuals living with serious mental illness.
Working with people with co-occurring disorders.
Providing social services to people from diverse cultures or communities.
With community-based case management.
