The Consumer Directed Community Supports (CDCS) Case Management Area of Health and Human Services seeks one Social Worker for the CDCS Team. In this position the CDCS worker will be responsible for ensuring that all CDCS cases transferred to Hennepin County meet waiver program/CDCS case management criteria. The case manager will work with participants in all aspects of CDCS case management, including plan approvals, insurance issues, appeal hearings, and related work. Provide case management duties and responsibilities as needed.
Location and hours:
This position is hybrid and will require occasional travel throughout Hennepin County and remote work as job duties require. Work hours are Monday through Friday, 8 a.m. to 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or be willing to locate to Minnesota or Wisconsin.
In this position you will:
Work with CDCS participants who transfer to Hennepin County to ensure documentation is current and accurate. Work with participants and collaterals to address case management needs until the case transfes to the on-going CDCS case manager.
Work with collaterals related to the transfer to ensure Federal and State criteria are met.
Complete plan reviews, decision memos, formal notices, and appeal packets.
Communicate with internal and external stakeholders via phone, email, and in-person in accordance with the case management role.
Complete timely documentation in accordance with program standards.
Review files to ensure all needed CDCS case transfers from other MN counties are complete prior to assigning to a case manager.
Need to have:
One of the following:
Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
Bachelor's degree in social work, psychology, sociology, or human services, and 18 semester (27 quarter) credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles or social welfare.
A valid driver's license and daily access to reliable transportation to travel primarily within/outside of the metro area to meet with clients in their residential settings and/or perform authorized off-site work duties.

Experience with:
Consumer Directed Community Supports (CDCS) program, related policy, and documentation.
Appeal documentation and participating at DHS CDCS appeals.
CDCS case management services or supervision to CDCS case managers
Home and Community based waiver programs (Community Alternative Care, (CAC), Alternative Care, Elderly (AC/EW), Developmental Disabilities (DD), Community Access for Disability Inclusion (CADI) and Brain Injury (BI).
Social work practice and ethics, including early intervention and prevention work.
Established history of promoting the representation and participation of diversity, inclusion, equity, accessibility and belonging in the workplace, community and in the work, you do each day.
Strong organizational and communication skills.
