DescriptionThe Behavioral Health (BH) area of Health and Human Services is seeking two Senior Social Workers to join the Adult Case Management program. These positions utilize person-centered and recovery practices to deliver case management services to adults (18+) who are court ordered to treatment for a serious mental illness and/or substance use disorder. The interdisciplinary teams are designed to promote stability and recovery. Team members work closely with the person to increase access to financial, housing, vocational, and treatment supports while prioritizing the person’s cultural and language needs.
Description of specialized case management teams:
The CIRT team provides case management services to adults who have been court-ordered for mental health and/or substance use disorder treatment. The team is responsible for assessment, coordination, and referrals to support residents’ access to community-based services that promote health and safety. This team works closely with community hospitals and substance use disorder treatment providers.
The HART team provides intensive case management to residents who are involved in both civil and criminal court related a finding of incompetence. Team members work closely with multiple institutions to meet residents’ needs, including state operated facilities. The team helps address system barriers to increase the person’s successful transition to the community, reduce hospitalization, and reduce incarceration.

Location and hours:
These positions are hybrid and will be performed both in-person at various locations in the community and remote as job duties require. Work hours are primarily days, Monday through Friday, with flexible hours between 8 a.m. - 5 p.m. No weekend, overnight or holiday shifts are required. While these positions are designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In these positions you will:
Provide adult targeted case management (including assessment, planning, referral linkage, follow up and crisis intervention) to individuals civilly committed due to mental illness or substance use and possibly involved in the criminal justice system.
Meet in-person, via the phone, and virtually to perform culturally responsive clinical assessments of strengths, needs, values, and preferences.
Mutually identify goals and strategies, including the most appropriate service and treatment options for recovery, health, and safety.
Work collaboratively with interdisciplinary team members for consultation, coordination, and coverage.
Manage timelines and tasks related to ongoing service and billing documentation, Functional Assessments, Individual Community Support Plans, obtaining Diagnostic Assessments, Correspondence, Referrals, Court Reports, and recommitment petitions.
Work collaboratively with the hospitals, Prepetition Screening Program, the County Attorney’s office, and civil and criminal courts to facilitate the least restrictive alternative while mitigating the person’s risk to themselves or others.
Refer to and coordinate community services that are preferred and responsive to the social, cultural, emotional, and care needs specific to the person.
Focus on psychiatric and/or addiction care, medication management, housing, vocational development, financial assistance, and other ongoing support needs
Provide in person services to people in a variety of service settings, including their homes, the community, and hospitals.
Act as a team lead to ensure referrals are initiated and completed to the most appropriate services and supports.
Need to have:
One of the following:
Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles of social welfare.
Bachelor's degree in social work, psychology, sociology, or human services, and at least 18 semester (27 quarter) credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
A valid driver's license and daily access to reliable transportation to travel primarily within the metro area to meet with persons in their residential settings and/or perform authorized off-site work duties and the ability to obtain a Hennepin County Driver's Permit to provide transportation as needed to essential services.
Nice to have:
Licensed as an Alcohol and Drug Counselor (LADC) in the State of Minnesota.
Experience:
Delivering person-centered, recovery-oriented, trauma-informed care and utilizing motivational interviewing tools and techniques.
Providing services with a focus on psychiatric and/or addiction care, outreach, medication management, housing, vocational development, financial assistance, and other ongoing support needs.
Serving people with civil commitment orders.
Conducting clinical assessments.
Navigating complex systems.
With discharge planning.
Finding and securing housing and supporting ongoing needs to maintain housing.
Working with medical assistance, waivers, and managed care organizations.
Knowledge of Hennepin County community resources and providers.
Ability to apply diversity, equity, and inclusion best practices to reduce disparities.
