NorthPoint Health & Wellness Center is seeking three experienced Staff Nurses to join our team. As a Staff Nurse, you will be an important part of the work that we do to support the health of our community. In these roles, you will handle walk-in patients who are seeking medical information and/or medical attention, perform telephone triage, and complete case management and direct patient care in an ambulatory family practice clinic, which includes pediatric, chronic disease and obstetrics patient care.
About these position types: These are full-time, benefit earning positions. These positions are internally classified as a Nurse, Staff. Click here to view the job classification specification. These positions are subject to the federal Centers for Medicare and Medicaid Services (CMS) Interim Final Rule on COVID-19 Health Care Staff Vaccination. As such, it requires the job offer to be conditional based on the candidate being fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. After a conditional job offer is made, the candidate must show proof of full vaccination prior to their start date. A COVID-19 vaccination card from the Center for Disease Control and Prevention (CDC), a vaccination record from your doctor’s office or a personal digital vaccination record constitutes sufficient “proof.” The candidate must also attest they are vaccinated by completing a form on the first day of employment. Reasonable accommodations to this vaccination requirement will be considered upon request by a candidate for medical or religious reasons as required by applicable law. In these positions, you will:
Provide triage by performing focused assessments of the signs and symptoms of illness and directing patients to an appropriate plan of care.
Schedule appointments, complete documentation, make or request referrals, and perform data entry of necessary information.
Work with multiple providers, staff, and collaborative partners in a culturally diverse environment to develop and maintain a plan of care to diverse and under-served patients.
Communicate and carry out provider care orders, answer questions, and provide limited health education.
Collect pertinent information and provide appropriate actions for patients who are seeking emergency care.
Provide case management, treatment follow-up and medication refills according to predetermined standing orders.
Provide health education and counseling to clients on disease conditions and health maintenance in areas such as prenatal education, chronic disease, family planning, STDs, and child development.
Provide professional nursing skills in patient assessment, intervention, and treatment, as well as RN support for the para-professional medical support staff.
Observe, record, and report patient's condition and reaction to drugs and treatments to physicians and dispense medication as directed.
Educate patient/family about diagnostic procedures, medications, nutrition and maintenance of health and wellness.
Be cross trained to work in other areas of the medical clinic.
Need to have:
Graduation from an accredited school of nursing.
A valid license, in good standing, as a registered nurse, or a permit to practice, issued by the State of Minnesota.
Basic Life Support (BLS) certification through the American Heart Association.
Current CPR certification.
Physical Requirements:
Full range of motion, manual and finger dexterity and eye-hand coordination.
Corrected hearing and vision to normal range.
May require some exposure to communicable diseases or bodily fluids.
Occasional medium work-exerting 20-50 pounds of force, frequently exerting up to 10 pounds of force to move objects.
Nice to have:
Three or more years of experience as a registered nurse in a health care setting, preferably primary care.
Experience:
Providing phone and face-to-face triage in a clinical setting.
With case management in an ambulatory family practice clinic, which includes pediatric, chronic disease, and obstetrics patient care.
Using the Epic electronic health record system to complete documentation in a timely and concise manner.
Ability to:
Work with multiple providers, staff, and collaborative partners with differing needs and opinions to improve the care to diverse, underserved, and at-risk populations with a variety of cultural and language barriers.
Be cross trained to work in other areas of the medical clinic.
Multi-task and work in a stressful environment.
Provide basic nursing skills techniques, such as: injections, IV therapy, minor procedures and staff supervision.
