Job Description: The RN Case Manager is the “hub of the wheel” to manage all areas of the patient care. Will collaborate with the Social Workers, CNA’s, Chaplin and Music and Massage Therapists. Will do pain and symptom management. The average length of stay in hospice care is 6 months. Where the patient calls “home” will be either long term facility or residence. Positions available in Rochester MN, Oakdale MN, Mankato, MN, and Bloomington MN Case load is 12 patients in a 60-mile radius, seeing on average 4-6 per day. They need live in their territory. Monday to Friday 8 AM to 5 PM with weekend and holiday rotations. When working a Saturday and Sunday they will have the Thursday off prior to the weekend or Tuesday off after the weekend. Have a large team and will be backup to the on-call team. ERM system is Home Base ( very user friendly). Base + additional Visits, Death pronounced, Admissions. Mileage reimbursement, if needed a tablet and phone and will provide scrubs. Very strong benefit package.
