DescriptionThe Long Term Services and Supports (LTSS) area of Health and Human Services is seeking a Social Worker to provide case management for individuals on waivers. In this role you will support individuals on the Community Alternatives for Disability Inclusion (CADI) and Brain Injury Waiver (BI) by assisting them towards greater independence by setting up and authorizing waivered services. The ideal Social Worker for this role will possess a strong customer service focus and excellent advocacy skills.

Location and hours:
This position is hybrid and will be performed at various locations in the community and remote as job duties require. Work hours are Monday through Friday, 8 a.m. - 4:30 p.m. No weekend, overnight, or holiday shifts are required.
While these positions are designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
Conduct phone and/or home visits in the community.
Complete the person’s Community Support and Services Plan annually.
Assess social and health needs of the person, assisting the individual in obtaining supports and services to help meet those needs.
Build partnerships with persons served, to identify and advocate for their needs, helping to reduce racial disparities while accessing services.
Work collaboratively with internal and external stakeholders via phone, email, and in-person in accordance with the case management role.
Complete timely documentation in accordance with program standards.
Develop knowledge of Department of Human Services (DHS), CADI and BI requirements and apply requirements of the DHS Community Based Services Manual while performing case management duties.
Need to have:
One of the following:
Licensed as a social worker by the Minnesota Board of Social Work.
Bachelor's degree in social work, psychology, sociology, human services, or a related field.
A valid driver's license and daily access to reliable transportation to travel primarily within the metro area to meet with persons in their residential settings and/or perform authorized off-site work duties and the ability to obtain a Hennepin County Driver's Permit to provide transportation as needed to essential services.
