The Access division of Hennepin County Human Services is seeking an Adult Access Social Worker, Senior to provide short-term assistance to individuals and/or families. This is a unique opportunity for a senior social worker to work side-by-side with Hennepin County Medical Center (HCMC) medical staff and social workers. This position will be located full-time at HCMC while being a part of the Adult Access team at Hennepin County. Supervision will be provided by Hennepin County with assistance and on-site support and supervision by HCMC.
Location and hours:
This position is hybrid and will be performed both on-site at the Hennepin County Medical Center (HCMC), 730 South 8th Street, Minneapolis, MN, 55415, and remote as job duties require. Work hours will be Monday through Friday, 8 a.m. – 4:30 p.m. or 8:30 a.m. -5 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

Provide short-term assistance to individuals and/or families to connect them with internal and/or community resources to help meet their needs; services will be provided in an ethical and culturally sensitive manner.
Partner with HCMC social workers and staff to assist with discharge and service planning in all units at HCMC.
Work with patients who may have mental health issues, chemical health issues, developmental and/or physical disabilities.
Work with patients who are experiencing homelessness and refer them to housing services and/or complete housing consultations for the Medicaid Housing Stabilization Service program.
Make appropriate referrals to county programs, including protective services.
Assist patients with accessing Rule 25 evaluations and treatment planning.
Complete Diagnostic Assessments to determine eligibility and access to services.
Assist with obtaining medical benefits, SNAP, SSI benefits, and SMRT (disability certification).
Document all activities in EPIC, ECF, and/or SSIS.
Need to have:
One of the following:
Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles of social welfare
Bachelor's degree or higher in social work, psychology, sociology, or human services, and 18 semester (27 quarter) credits of graduate work in the above areas, and three years of full-time supervised experience as a social worker.
Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
