DescriptionCome help us elevate the lives of our residents! We are looking for an Adult Mental Health Initiative (AMHI) Service Coordinator with expertise in adult mental health service delivery to join our Behavioral Health service area to support Medical Assistance (MA) eligible residents toward the appropriate level of care as well as provide education and tools to community partners toward maximized utilization. This position will be a member of an interdisciplinary team of behavioral health service delivery and administrative staff. The team administers and oversees MA eligible services serving adults 18+ living with severe and persistent mental illness (SPMI).

Location and hours:
This position is remote. Remote workers may be required to come on-site an average of 0 to 12 days a year for work meetings, trainings, or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training). Supervisors will provide as much advance notice as practicable. Work hours are typically 8 a.m. - 4:30 p.m., Monday through Friday. While this position is designated as remote, based on current requirements hires must reside in Minnesota or Wisconsin.
New employees who are hired into remote positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
Conduct person-centered comprehensive assessments toward referral and eligibility of applicants to AMHI funded services and make appropriate community resource referrals on behalf of the client.
Provide coordination, education and support to contracted agencies regarding AMHI funded and alternative adult mental health services.
Engage county residents living with SPMI in developing community best practices around service delivery.
Assess community need for residents with a person-centered and culturally responsive approach to determine a plan of action.
Increase responsiveness and collaboration with partners including courts, law enforcement, and community providers.
Increase the quality of care to residents, stability, and safety in communities.
Analyze assessed data and assist the applicant in identifying relevant goals along with specific supports that will enhance a person’s ability to meet goals.
Coordinate access to Medicaid waiver programs and MA services.
Document data and case activities in state and county computer-based programs.
Need to have:
One of the following:
Bachelor's degree or higher in social work, psychology, sociology, human services, or an equivalent field.
Licensed as a social worker by the Minnesota Board of Social Work.
Nice to have:
Experience providing mental health case management, intake, and/or care coordination.
Two or more years of experience providing community-based adult mental health services.
Licensed mental health professional; LAMFT, LPC, LGSW, LMFT, LPCC, LICSW.
Excellent interpersonal, organizational, computer, and person-centered interviewing skills.
