Hennepin County's Office of Multi-Cultural Services within the Initial Contact and Access area of Health & Human Services is seeking a Case Management Assistant to be part of the team. This position will assist with supporting health care access for families in the Child Services/Child Protection system by connecting them to a variety of health care programs and community resources. This is done by receiving medical assistance referrals from the Child Protection team, Social Services Front Door, and Health Care Navigators.
Location and hours:
This position is hybrid and will be performed both on-site at the South Minneapolis Human Service Center at 2215 East Lake Street, Minneapolis, MN 55407 and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. and there will be an occasional need for working evenings and weekends. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
In this position, you will:
Assist families with understanding the various health care options available to them.
Provide application assistance, troubleshoot eligibility barriers, and secure verifications.
Conduct the Broader Needs Assessment and child teen check-up referrals
Participate in efforts to address disparities in Hennepin County communities.
Document efforts in applicable databases.
Need to have:
One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Nice to have:
Experience:
Working with child protection services and programs.
Using Social Services Information System (SSIS), Electronic Case Files (ECF) and/or other database systems to perform data entry where accuracy is essential.
Ability to:
Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
Work both independently and collaboratively as an effective team member.
Make sound, independent judgment.
