Hennepin County Public Health (HCPH) is seeking a Community Based Nursing Supervisor to oversee and direct the work of two maternal and child health program teams within the Family Health area. This position will provide supervision, oversight, and leadership to the Caring Connections 4 Kids and Follow Along program teams. This position will lead diverse teams of public health nurses and administrative professionals to conduct childhood screening and assessment services, collaborate closely with internal and external program partners, connect with community providers and services for referrals, maintain program grants relationships and submit required reporting, as well as overseeing daily operations and continuous quality improvement.

Location and hours:
This position is hybrid andwill be performed both on-site at community locations and remote as job duties require. Work hours are Monday through Friday, 8 a.m. to 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
In this position, you will:
Oversee planning, implementation, and day-to-day operations of the programs Caring Connections 4 Kids and Follow Along.
Monitor program and screening results, data, identify trends, and address opportunities and barriers.
Supervise and train program staff, establish performance expectations, evaluate team and individual performance results, and provide coaching for development and improvement. Lead and facilitate team and individual check-in meetings. Recruit and onboard new staff.
Conduct a small caseload of screenings with families on an ongoing basis and serve as backup for screening and outreach.
Represent the family health area and programs on committees and task forces, act as a liaison with other county programs, state and local units of government, contracted providers, hospitals and clinics, and community stakeholders.
Prepare reports and other written materials for funders and HCPH leadership.
Facilitate and maintain the program Health and Racial Equity plans, ensuring implementation and continuous quality improvement.
Need to have:
Bachelor's degree or higher from an approved school of nursing and two or more years of professional nursing experience.
Licensure as a Registered Nurse issued by the state of Minnesota.

Supervising and leading diverse teams to develop goals and projects, implement work plans, and evaluate results.
Project management and program planning.
Working with Family Home Visiting or Child Protection programs, data management for grant reporting and tracking FHV provider capacity, outreach and referrals.
Working with clinical providers, building relationships with community stakeholders, working with multidisciplinary teams, and facilitating strategic discussions.
Ability to:

Communicate effectively in both verbal and written form.
Demonstrate understanding of cultural differences and the impacts of racism on service provision, organizational effectiveness, and on relationships with staff, partners, and those we serve.
Use Microsoft or Google programs including Word/Google Docs, Excel/Google Sheets PowerPoint/Google Slides, and Outlook.

Public health approaches and practices.
The impacts of racism on health and the social determinants of health.
