Hennepin Health is seeking a Community Nurse to join the Social Service Community team. This position will work with a team of social workers, other nurses, and community health workers to address the social determinants of health of our Hennepin Health members. This role will largely focus on the most medically and socially fragile members of our population, to address their needs in a trauma informed and member-centric way. This is a community-based, dynamic position often calling for creative solutions to difficult problems that are outside of traditional clinic-based interventions. The person filling this role must be flexible, creative and have an understanding of marginalized populations, the unique health conditions affecting them and the social context in which they develop.
In this position, you will:
Function independently without on-site supervision.
Provide health education to individuals.
Complete visits in the home, clinic, hospital and/or community.
Work directly with marginalized populations to address the immediate needs that affect their overall health and well-being.
Need to have:
Graduated from an accredited school of nursing.
Possession of a valid license, in good standing, as a Registered Nurse, or a permit to practice, issued by the State of Minnesota.
Current CPR certification.
Six months nursing experience in a related area.
A valid driver's license and the ability to obtain a Hennepin County driver's permit.
Access to reliable transportation to travel to multiple sites within Hennepin County.
Nice to have:
Public Health Nurse (PHN) certification.
Experience:
Working in community settings.
Working with Epic electronic health records.
In care coordination or case management.
Ability to:
Understand social safety net programs.
Work independently in various settings including places such as clinics, hospital rooms, community areas and homeless shelters.
Counsel and instruct individuals in a state of stress.
Act as advocate for client when dealing with other service providers.
Interpret and communicate verbal information on a one-to-one basis and communicate complex medical and technical information to physicians, clients/patients, nursing staff, and health care team.
Read and interpret client/patient charts, lab reports, and physician notes.
Plan and organize activities to accomplish patient centered and patient directed goals.
Maintain accurate records of client/patient visits.
Trauma informed approach to work.
