DescriptionNorthPoint Health & Wellness Center is seeking an experienced Psychiatric Nurse Practitioner. This position will work on a multidisciplinary team to provide psychiatric support services to behavioral health clients. This care includes client evaluation, appropriate prescribing of medications, and medication management.

Location and hours:
This position is hybrid and will be performed both on-site at NorthPoint Health & Wellness Center at 2220 Plymouth Avenue North, Minneapolis, Minnesota 55411, and remote as job duties require. Work hours will typically be Monday through Friday, between 8:30 a.m. and 5 p.m. While this position is designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

This position is subject to the federal Centers for Medicare and Medicaid Services (CMS) Interim Final Rule on COVID-19 Health Care Staff Vaccination. As such, it requires the job offer to be conditional based on the candidate being fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. After a conditional job offer is made, the candidate must show proof of full vaccination prior to their start date. In this position you will:
Conduct initial and on-going psychiatric diagnostic assessment of patients' mental health, physical health, and psychosocial functioning.
Prescribe medications following the clinic’s policy, formulary, and relevant regulations of the State.
Complete accurate and timely documentation of observations, interventions, plans of care, referrals, and follow up.
Assess and evaluate mental health and medical history.
Provide client and family with health education.
Make appropriate referrals to other health professionals and community agencies.
Need to have:
Graduation from an accredited school of nursing.
One of the following:
Possession of a valid license as a Registered Nurse issued by the State of Minnesota and a National Certification as a Nurse Practitioner (CNP).
Possession of a valid license as an Advanced Practice Registered Nurse issued by the State of Minnesota.
Nice to have:
Master's degree or higher in Psychiatric or Mental Health Nursing from an accredited school of nursing.
Nine years or more of experience as a Nurse Practitioner.
Experience:
Working in a psychiatric/mental health setting.
Utilizing Epic or another similar electronic health record system.
Working with populations diverse in race, ethnicity, age, gender, identity, ability, and sexual orientation.
Working independently and interdependently with professionals from other disciplines.
Obtaining health history and performing psychiatric assessment and treatment.
Coordinating and monitoring health maintenance and preventative care.
Providing consultation to medical, nursing, and behavioral health personnel.
Excellent communication skills, both verbally and in writing.
Effective problem-solving skills and ability to adapt to an ever-changing clinic practice environment.
Ability to evaluate the Impact of Social Determinants of Health on clients and families.
