Description The Housing Stability area in Health and Human Services is seeking a Coordinated Entry System Planner. The Coordinated Entry System (CES) is a dynamic and nuanced system that assesses and matches eligible households to homeless dedicated housing opportunities using prioritization of eligibility and client choice. This planner will focus on mezzo-level planning to ensure the system is functioning as it should on a day-to-day basis, solve issues, fill gaps and support community providers. The CES Planner will work on a 5-person CES team to create, revise, maintain policies and procedures, and business processes to promote community engagement, client choices, and build equity in CES system. Current Hennepin County employees who refer a candidate hired into an open competitive position may be eligible for a $500 referral bonus. Location and hours: This position is hybrid and will work primarily remote with occasional travel to various community sites within Hennepin County. Work hours are 9 a.m. - 5 p.m. Monday through Friday. While this position is designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin. New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment. In this position, you will:
Take on complicated communication exchanges, identify when individual issues roll up into system issues, and escalate to the appropriate CES team member as needed.
Identify, evaluate, and efficiently address needs and gaps for the area's Continuum of Care (CoC) Coordinated Entry System (CES).
Engage with people experiencing homelessness on a regular basis in a person-centered, trauma informed way to implement their feedback and voice into the system.
Analyze and evaluate CES processes, information flow, and outcomes for operational effectiveness, making recommendations to the leadership committee and internal team for needed interventions, shifts or changes.
Lead system change and continuous improvement projects by creating, revising, and maintaining CES policies and procedures, and business processes; may manage projects to completion.
Regular interaction with the Homeless Management Information System (HMIS), working with the Institute for Community Alliances (ICA) and internal partners to increase efficiency, ease of use and support for excellent data quality within HMIS for the CES workflows.
Provide back up support for Referral Coordinators, data collection for Chronic Index and send chronic data to providers.
Monitor the CES inbox daily and answer questions from clients and partners.
Plan and launch supportive internal processes that will support overall system efficiency – this could include adding pertinent documents to client records including income and housing/criminal background info.
Need to have:
One of the following:
Bachelor's degree in any field and a minimum of two courses in research methods, analysis, statistics, project management, planning or evaluation and 18 months or more of relevant planning analyst experience.
Bachelor's degree in one of the fields listed below and one year or more of relevant planning analyst experience.
Master's degree or higher in business administration, public administration, research methods, statistics, behavioral/social science, public health, computer science/MIS, management science, urban/city planning or a field appropriate to the area of specialization.

Experience:
Working within a homeless response system and coordinated entry system.
Working with homeless and housing providers.
Working with HMIS including entering data and running reports.
Creating, reviewing, and revising policies and procedures.
Facilitating, coordinating, and planning community meetings.
Working with state databases such as ECF, MAXIS, etc.
Knowledge of:
Trauma informed care, client centered care, and strengths-based approaches.
Principles and practices of planning and program evaluation.
Ability to:
Communicate with and understand the problems faced by individuals from all cultural backgrounds and economic levels.
Develop and maintain effective working relationships with staff, residents, and agencies.
Stay organized and meet deadlines while managing multiple projects and daily tasks.
