DescriptionThe Long Term Services and Supports (LTSS) area of Human Services and Public Health is hiring 3 Program Managers. One program manager to lead case management, one to lead initial assessments, and one to lead annual assessments. LTSS provides initial and annual MnCHOICES assessments, ongoing case management for older adults and persons with disabilities (including intellectual and developmental, physical, behavioral health, brain injuries, chronic complex medical, and other disabling chronic conditions), and licensing for adult foster care.
These leadership positions will provide management and supervision of Community Based Nursing Supervisors and Social Work Unit Supervisors who lead teams of employees. These positions will work collaboratively with the LTSS leadership team to ensure program integrity, employee engagement, best practices in case management and assessments, reduction in disparities, and meeting identified outcomes. Additionally, it is expected these positions will work in partnership with other department leaders to ensure the county system is accessible and responsive to the persons we serve.
Case Management Program Manager: will share leadership of the case management area of LTSS, this position will have a focus on supports to individuals with intellectual and developmental disabilities including youth in out of home placement. This manager will collaborate with the other case management program manager to align case management practices across operated and contracted case managers when applicable. The individual selected will develop and monitor contracted services for people with intellectual and developmental disabilities delivered by community-based providers.
Initial Assessment Program Manager: will share leadership of the initial assessment area of LTSS to lead programs that work with all populations and programs. This manager will partner with other Initial Assessment Program Managers to align initial assessment practices for initial eligibility determination and providing equitable access to programs and services.
Annual Assessment Program Manager: will share leadership of the annual assessment area of LTSS to lead programs that work with all populations and programs. This manager will partner with Annual Assessment Program Managers to align re-assessment practices for waiver recipients when applicable, as well as collaborate with the LTSS Program Management team on processes and working agreements to best support the people we serve.

Location and hours: These positions are remote. Remote workers may be required to come on-site an average of 0 to 12 days a year for work meetings, trainings, or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training). Supervisors will provide as much advance notice as practicable. Work hours are Monday through Friday typically between 8 a.m.- 4:30p.m with flexibility. There is potential for some afterhours meetings when necessary. While these positions are designated as remote, based on current requirements hires must reside in Minnesota or Wisconsin. New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment. Plan, organize, evaluate, and direct the designated operations through shared decision making with LTSS leadership team.
Determine needs for improvements that will enhance program integrity and develop resources and plans to meet such needs and evaluate outcomes.
Manage the preparation of periodic reports of program performance, areas for improvement, and progress towards results.
Provide management and clinical input on best practices and develop and monitor contracted services with community-based providers.
Provide assistance to unit supervisors in evaluating employee performance, resolving employee discipline, and other supervisory issues.
Work collaboratively with LTSS leadership team members to provide back-up and/or assume administrative responsibility for service area operations when designated by the area manager.
Facilitate collaborative working relationships with other service areas to promote the most integrated and holistic service approaches for individuals and their families.
Cooperate and collaborate with the Minnesota Department of Human Services, other county departments, public and private agencies, advisory boards, community groups, and other organizations to improve program integrity and outcomes.
Participate in the preparation of budget proposals and monitor expenditures and revenues.
Analyze legislation, develop legislative proposals, and recommend changes in policy.
Need to have: One of the following:
Seven or more years of supervisory experience in an area related to the position.
Bachelor's degree in behavioral or social science, business administration, public administration, public health or a related field and three years or more of supervisory experience in an area related to the position.
Master's degree or higher one of the above fields and two years or more of supervisory experience in an area related to the position.
Nice to have:
Knowledge of assessment, support planning, and service development for older adults, and for populations with brain injury, mental health, complex medical, developmental, and physical disabilities.
Experience providing or supporting teams delivering case management services and/or conducting assessments.
Experience:
Providing excellent communication skills and presenting complex information in large and small group settings.
Fostering effective working relationships with internal and external contacts at all levels of responsibility.
Working with Microsoft Office applications.
Providing project management that demonstrates strong leadership skills, personal initiative, and effective decision making with resulting outcomes that are in line with strategic objectives.
