DescriptionCome help us elevate the lives of our residents! We are looking for Senior Social Workers to complete annual assessments in our Long-Term Services and Supports (LTSS) service area of Health and Human Services utilizing the MnCHOICES assessment tool. These Senior Social Workers will be members of an interdisciplinary team of Public Health Nurses and Social Workers. The team administers Medicaid waiver and state programs that in addition to serving seniors, serve individuals with brain injury, mental health, complex medical, developmental, and physical disabilities. Current Hennepin County employees who refer a candidate hired into an open competitive position may be eligible for a $500 referral bonus. For more information visit employee referral program. Location and hours: These positions are hybrid and will be performed both on-site at a variety of Hennepin County offices, at community locations to meet with people supported by LTSS programs, and remote as job duties require. Work hours are typically Monday through Friday, 8 a.m. - 4:30 p.m. While these positions are designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin. New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment. In these positions, you will:
Conduct person-centered comprehensive social, health, medical, and psychiatric assessments using the MnCHOICES assessment tool.
Complete referrals for service planning and crisis intervention services.
Determine applicants’ risk of institutional care and their level of care according to state and federal criteria.
Analyze assessed data and assist the applicant in identifying relevant goals along with specific supports that will enhance a person’s ability to meet goals.
Determine and make appropriate community resource referrals on behalf of applicant.
Maintain a broad knowledge base related to payer sources and community-based services.
Coordinate access to Medicaid waiver programs and Medical Assistance (MA) Home Care services.
Redetermine eligibility of applicants to waivers and Personal Care Assistant (PCA) services according to availability.
Document data and case activities in state and county computer-based programs.
Assist supervisors with quality assurance, continuous improvement activities, and case audits.
Act in a lead mentor role for onboarding of new staff.
Need to have:
One of the following:
Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles of social welfare.
Bachelor's degree or higher in social work, psychology, sociology, or human services, and 18 semester (27 quarter) credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
A valid driver's license and daily access to reliable transportation to travel to a variety of Hennepin County offices and community locations, and to visit clients in residential settings.
Nice to have:
One or more years of experience providing home and/or community-based services.
Experience:
Providing case management as a professional social worker.
Working with payer sources for home and community-based services and knowledge of existing community-based services.
Working with Medicaid waiver programs and MA Home Care services.
Supervising, mentoring, or leading a team.
Strong interpersonal, organizational, computer, and person-centered interviewing skills.
