Description
The Behavioral Health service area of Hennepin County Human Services is seeking to hire a Case Management Assistant to support the work of the Police Embedded Social Work Program. The Police Embedded Social Work Program provides early identification and intervention for individuals with mental health, medical, and substance use needs who have contact with emergency response systems such as law enforcement, fire, or EMS. The individual in this position will accompany assigned primary worker on community outreach visits, assist program participants with applying for benefits, navigating county systems, arranging and/or providing transportation to appointments in the community and entering program data into county systems.

Location and hours:
This position is hybrid and will be located at the Behavioral Health Center at 1800 Chicago Avenue South Minneapolis, MN 55405. Travel throughout the community to client homes, health care facilities, and other locations providing client resources and services will be required with remote work as job duties require. Work hours will be Monday through Friday 9 a.m. to 5:30 p.m. with some limited evening hours. Transporting clients is often required. While this position is designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
Assist in the coordination of services, applying for benefits, navigating county systems and providing community resources as needed.
Arrange and/or provide transportation to appointments in the community.
Document case notes utilizing state and county systems.
Complete file clearances, open intake and assessment workgroups in online databases.
Track inventory of bus tokens, supplies and resources.
Conduct community outreach visits.
Need to have:
One of the following:
Two years of responsible, paid social service experience providing direct client service
A two-year human services certificate
Note: College coursework in the behavioral sciences can be substituted for the required work experience.
A valid driver's license, the ability to obtain a Hennepin County drivers permit, and daily access to reliable transportation to travel to service sites and residences within Hennepin County.

Serving individuals living with a mental health condition, substance use disorder, homeless/housing instability and/or involved in the criminal justice system.
Knowledge of:
Diversity of cultural and lifestyle experiences, beliefs and practices relating to spiritual, mental, and physical health, community, family structure and dynamics and disparities.
County and community resources for mental health, substance use disorder, social services, and basic needs.
County and state record systems.
Basic knowledge of Hennepin County financial assistance programs and navigating county systems for single adults and families.
Ability to:
Communicate effectively and clearly, both verbally and in writing.
Demonstrate strong interpersonal, organizational, time management and computer skills.
Effectively interact with people from diverse backgrounds including efforts towards disparity reduction.
