The Housing Stability area in Health and Human Services is seeking a Coordinated Entry Priority List Coordinator to join their team. This position will work with housing providers to make housing referrals via the Homeless Management Information System (HMIS) and CES Connect, work with various stakeholders in the housing and homelessness community to clearly communicate the work and mission of the Coordinated Entry System (CES), and support assessment functions by monitoring the CES inbox to ensure client needs and preferences are met in a timely manner.
Location and hours:
This position is hybrid and will be performed both on-site at the South Minneapolis Human Service Center at 2215 E Lake Street, Minneapolis, MN 55407 and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
In this position, you will:
Receive referral request forms from housing providers and makie housing referrals via the Homeless Management Information System (HMIS) and CES Connect.
Manage vacancy reporting, priority list management, and referral functions to ensure that clients are appropriately prioritized, matched and referred to homeless housing vacancies as they arise in the community.
Administratively support assessment functions and CES inbox to ensure client needs and preferences are promptly, regularly, respectfully, consistently and accurately determined.
Monitor priority list and other data in HMIS to manage client and program level data including assessments, priority lists, vacancy reporting, referrals, and referral outcomes.
Perform system maintenance within HMIS regularly.
Develop and maintain strong working relationships with referring and receiving agencies including comprehensive knowledge of program types and provider attributes.
Work closely with the other CE Priority List co-manager and CES Planners to ensure that system is functioning as it should.
Work with various stakeholders in the housing and homelessness community to clearly communicate the work and mission of the Coordinated Entry System.
Support the CES Leadership Committees by providing data, reporting trends, and contributing “boots on the ground” priority list manager perspective.
Need to have:
One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Nice to have:
Experience:
Working with people experiencing homelessness or vulnerable populations.
With program design, start-up, and evaluation.
Using the HMIS system.
Effective interpersonal, analytical, project management, oral/written communication, and negotiation skills.
Knowledge of:
Homeless and housing providers in Hennepin County.
Housing funding sources.
Ability to:
Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
Work both independently and collaboratively as an effective team member.
Make sound, independent judgment.
