Hennepin County Public Health's Red Door Clinic is seeking a candidate for the HIV Care Navigator and Outreach Specialist position. This position will create and maintain relationships with newly diagnosed and out of care individuals living with HIV to support and ensure establishment and retention in HIV care and develop outreach strategies to build intentional relationships with community members and reach intended populations at disproportionate risk for HIV.
The Public Health Clinic (PHC) is a respected leader in providing sexual health services, comprehensive testing & treatment for tuberculosis, and refugee health exams. The PHC is home to the Red Door Clinic, Minnesota’s largest HIV and sexually transmitted infection clinic. The Red Door Clinic has been providing culturally sensitive and sexually affirming services for the last 50 years. The mission of PHC is to reduce health disparities, increase access, and promote health across communities with specialized services that value evidence-based, client-centered care to create empowered and healthy communities.
Location and hours: This position is hybrid and will be performed on-site at the Public Health Clinic located at 525 Portland Avenue, Minneapolis, MN 55415 and remotely as job duties require. Work hours are Monday, Tuesday, Wednesday, and Friday, 8 a.m. to 4 p.m., and Thursday 10 a.m. - 4 p.m. This position will require working some evening shifts when the clinic resumes evening hours on Mondays and Wednesdays until 7:30 p.m. No weekend, overnight or holiday shifts are required. While this position is designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

This position is subject to the federal Centers for Medicare and Medicaid Services (CMS) Interim Final Rule on COVID-19 Health Care Staff Vaccination. As such, it requires the job offer to be conditional based on the candidate being fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. In this position, you will:
Address barriers to HIV care and refer newly diagnosed and out of care individuals living with HIV to core medical and supportive services, educate about HIV, risk reduction, and treatment.
Organize client transportation; accompanying clients to their first HIV medical care appointment to ensure care linkage; collaborating with nurses and social workers to verify care status; and providing necessary follow-up with clients on missed initial HIV care appointments.
Provide client centered, non-judgmental HIV testing, sexual health education and counseling, care coordination, and referral services in clinic and community settings for populations disproportionately impacted by HIV, including African American MSM, Latinx MSM, transgender individuals, other LGBTQ+ communities, and people who inject drugs.
Represent Red Door Clinic by collaborating with outside community groups, HIV service organizations, and health care providers.
Collaborate with other Red Door team members in developing team goals, measuring performance outcomes, facilitating effective referral coordination, helping write work plans, and collecting, tracking, and reporting program data and service delivery outcomes that demonstrate our work.
Assist with planning, designing, and co-facilitating a support group or other forms of psychosocial support for individuals who are living with HIV.
Need to have:
One of the following:
Bachelor's degree or higher in public health, nursing, healthcare, behavioral/social science, or an approved related field.
Four or more years of experience providing health education, counseling, outreach and referral services for individuals living with HIV.
A valid driver's license and daily access to reliable transportation to attend meetings and visit community sites throughout Hennepin County.

Provide a supportive space that allows clients to begin to process a new diagnosis.
Exercise independent judgment and initiative in performing assigned duties and addressing clients’ needs.
Communicate effectively, both verbally and in writing.
Work independently as well as part of a larger team.
