Better Together Hennepin (BTH) is a Hennepin County Public Health initiative to promote adolescent sexual health and wellbeing. BTH is seeking a Project Coordinator to join the Health Mentor Model Project (HMMP) team. The goal of the project is to improve the optimal health of adolescents and reduce teen pregnancy and sexually transmitted infections in Hennepin County communities with the greatest needs and disparities. The Health Mentor Model (HMM) is an innovative, evidence-based, systems thinking, community-led, culturally, and linguistically responsive model in partnership with eleven high schools and four community clinic organizations in six geographic communities in Hennepin County.
In this position, you will:
Assist in data collection, performance measures, and creating bi-annual reports.
Coordinate assessment and evaluation activities, including youth and parent listening sessions.
Coordinate project trainings as needed.
Assist with project communications, including quarterly e-newsletter and social media posts related to teen pregnancy prevention, STIs and optimal health.
Coordinates and provide oversight to Youth Leadership Council activities conducted by community partner agencies.
Work with local clinics to increase capacity of adolescent friendly services.
Engage in outreach for the HMMP.
Conduct observation of effective program sessions in compliance with guidance from federal funder.
Need to have:
One of the following:
Bachelor's degree or higher in public health, nursing, healthcare, behavioral/social science, or an approved related field.
Four years or more of experience implementing or supporting the implementation of evidence-based adolescent sexual health programming.
Nice to have:
Experience inputting, retrieving, exporting, and modifying information and data stored in computerized systems and/or applications.
Knowledge of community resources and social services available to clients.
Ability to:
Exercise independent judgment and initiative in performing assigned duties and addressing clients’ needs.
Communicate effectively, both verbally and in writing.
Work independently as well as part of a larger team.
Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
