Hennepin County's Office of Multi-Cultural Services within the Initial Contact and Access area of Health & Human Services is seeking two Case Management Assistants who are bilingual in Somali to be a part of the team. These positions will assist with supporting health care access for families in the Child Services/Child Protection system by connecting them to a variety of health care programs and community resources. This is done by receiving medical assistance referrals from the Child Protection team, Social Services Front Door, and Health Care Navigators.
Location and hours:
These positions are hybrid and will be performed both on-site at the South Minneapolis Human Service Center at 2215 East Lake Street, Minneapolis, MN 55407 and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. There will be an occasional need for working evenings and weekends. While these positions are designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
In these positions, you will:
Assist families with understanding the various health care options available to them.
Provide application assistance, troubleshoot eligibility barriers, and secure verifications.
Conduct the Broader Needs Assessment and child teen check-up referrals.
Participate in efforts to address disparities in Hennepin County communities.
Document efforts in applicable databases.
Need to have:
One of the following:
Two years or more of responsible, paid social service experience providing direct client service.
A two-year human services certificate.
Two years or more of college coursework in the behavioral sciences.
Ability to communicate fluently in Somali, in addition to English, both verbally and in writing.
Nice to have:
Experience:
Working with child protection services and programs.
Using Social Services Information System (SSIS), Electronic Case Files (ECF) and/or other database systems to perform data entry where accuracy is essential.
Ability to:
Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
Make sound, independent judgment.
Work both independently and collaboratively as an effective team member.
